29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): a) Allen Plumbing and Heating, LLC, 1933 E 16th Street, Greeley, CO; On and before August 13, 2022 Allen Plumbing and Heating, LLC, as the controlling employer, did not ensure that employees of GoldStar Excavation and Sewer, were safe while conducting water line repair in a trench, by conducting frequent and regular inspections of the job site by a competent person in order to prevent and detect safety and health hazards. The hazardous trench caved-in while workers were in the trench burying two, one of whom was fatally injured.

The injury and illness records were not provided for 2022 and the injury and illness summary report was not provided for 2021.
