On or about August 16, 2022, at the site location of 499 N. Sandusky St. Tiffin, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards, such as but not limited to falls and live electricity during job site operations, decreasing the probability of employee injury and/or illness.

Two employees tasked with attaching shingles with a pneumatic nail gun without the benefit of eye protection were exposed to eye injuries.
