(Construction reference 29 CFR 1926.602(d)):  a) At or near 383 Main St, Little Falls NJ - Building # 8 - Front - 1st  Floor: Employees were exposed to fall and struck by hazards as the All Terrain Forklift was operated by an individual that had not been trained and evaluated in the safe operation of the equipment, on or about 3/2/22.  b) At or near 383 Main St, Little Falls NJ - Building # 8 - Back - Roof: Employees were exposed to fall and struck by hazards as the All Terrain Forklift was operated by an individual that had not been trained and evaluated in the safe operation of the equipment, on or about 3/5/22.
