29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Jobsite, 129 Macleod Drive, Valencia, PA: On or about August 12, 2022, an employee suffered an amputation during concrete delivery operations because a broken bracket inside the drum created a hazardous condition and was not reported for a week. The employer should re-instruct each employee in the recognition and avoidance of unsafe conditions and ensure that employees utilize safe procedures while rinsing out the mixer drum and follow the steps for Cleaning the Drum and the Chutes located in the Terex Front Discharge Mixer Operations Operator's manual.
