29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about August 17, 2022, on site of a residential structure located at 780 NW 141st Street, Miami, Florida, employees performing roof tile installation work were exposed to a fall hazard of approximately 10.7 feet while climbing up and down an extension ladder leaning on the house with the ladder's rails not being at least 3ft above the upper landing.
