29 CFR 1926.652(b)(2):Maximum allowable slopes, and allowable configurations for sloping and benching systems, was not determined in accordance with the conditions and requirements set forth in Appendices A and B to this subpart. On or about August 17, 2022, employees were exposed to struck-by and crushed-by hazards when installing storm boxes in an excavation exceeding a depth of 5 feet with a benching system that did not meet the allowable configurations.
