On or before August 17,2022 at a work site located at 8490 East Wheeling Road, Norwich Ohio 43767: The employer failed to ensure employees had a stairway, ladder, ramp or other safe means of egress from a trench. Employees were working in a 5 foot 6 inches deep trench, without any means of egress, thereby being exposed to crush by hazards.

29 CFR 1926.651(j)(2):Employees shall be protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations. Protection shall be provided by placing and keeping such materials or equipment at least 2 feet (.61 m) from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. a. On or before August 17,2022 at a work site located at 8490 East Wheeling Road, Norwich Ohio 43767: The employer failed to ensure the spoil pile was at least 2 feet from the edge of the excavation. Employees were working in a 5 foot 6 inches deep trench where the spoil pile was placed directly at the edge of the excavation, thereby being exposed to crush by hazards.
