29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) Workers were exposed to struck-by hazards at a worksite at 5929 Rockhill Rd, Kansas City, MO 64110. This was last demonstrated on or about August 18, 2022,  when employees were permitted to operate pneumatic nail guns at 70 to 120 psi without wearing impact-resistant eye protection while roofing a residential building at or about 8:10 AM.  b) Workers were exposed to struck-by hazards at a worksite at 5929 Rockhill Rd, Kansas City, MO 64110. This was last demonstrated on or about August 18, 2022,  when employees were permitted to operate pneumatic nail guns at 70 to 120 psi without wearing impact-resistant eye protection while roofing a residential building at or about 4:53 PM.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest system, nor were an employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Workers were exposed to fall hazards at a worksite at 5929 Rockhill Rd, Kansas City, MO 64110. This was last demonstrated on or about August 18, 2022, when workers were engaged in roofing activities. Workers removing decking and roofing materials were exposed to fall hazards ranging from 9 to 14.8 feet from the eve of the roof to the ground below on or about 8:10 AM. No guard rails, safety net system(s), or personal fall arrest system(s) were utilized to prevent falls.  b) Workers were exposed to fall hazards at a worksite at 5929 Rockhill Rd, Kansas City, MO 64110. This was last demonstrated on or about August 18, 2022, when workers were engaged in roofing activities. Workers applying decking and roofing materials were exposed to fall hazards of 18.9 feet from the eve of the roof to the ground below on or about 4:53 PM. No guard rails, safety net system(s), or personal fall arrest system(s) were utilized to prevent falls.

29 CFR  1926.502(d)(15)(i)):  Anchorages used for attachment of personal fall arrest equipment were not designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two:    Workers were exposed to fall hazards at a worksite at 5929 Rockhill Rd, Kansas City, MO 64110. This was last demonstrated on or about August 18, 2022, when workers were engaged in roofing activities.  a) Workers roofing a residential home were exposed to fall hazards of  14.8 feet from the eve of the roof to the ground below. Anchorage being used by workers was not installed per the manufacturer as part of a complete fall arrest system utilized to prevent falls. The anchorage base was not fully opened and secured using the appropriate number of screws.  b) Workers roofing a residential home were exposed to fall hazards of  14.8 feet from the eve of the roof to the ground below. Anchorage being used by workers was not used per the manufacturer as part of a complete fall arrest system utilized to prevent falls. More than one person was using the anchorage simultaneously for personal fall protection.

This was last demonstrated on or about August 18, 2022, when workers were engaged in roofing activities. Workers removing decking and roofing materials were exposed to fall hazards 9 feet from the eve of the roof to the ground below. Workers were permitted to carry sections of roof decking material while ascending an extension ladder to gain access to the roof.
