29 CFR 1910.178(n)(6):Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel: At a storage facility located at 11893 Lime Kiln Drive, Neosho MO on 8-18-2022 the employer did not ensure the forklift driver looked in the direction of travel and maintain a clear view of path of travel and surroundings exposing employees to being caught-in, crushed, and struck-by hazards. This resulted in an employee being struck by and crushed between forklifts.
