29 CFR  1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  On or about August 18, 2022 the employer Andres Rojas at jobsite located at 1355 Roberts Cir. Waterville OH 43566, did not have a plan or a program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls and struck by hazards during job site operations, decreasing the probability of employee injury or illness.

29 CFR  1926.1060(a):The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:  On or about August 18, 2022 the employer Andres Rojas at jobsite located at 1355 Roberts Cir. Waterville OH 43566, did not provide a training program for each employee potentially exposed to fall hazards while using a portable ladder to enable each employee to recognize the hazards of falling from the ladder and the procedures to be followed in order to minimize these hazards. Five employees using the single aluminum extension ladder to access the roof to perform roofing work were exposed to fall hazards 11 feet or greater to the ground below.

Waterville OH 43566, did not ensure the side rails of one portable aluminum extension ladder was extended at least three feet above the landing surface. Five employees were exposed to falls from approximately 11 feet or greater from the top of the second (roof) level to the decking below while using the ladder to access the second (roof) level to perform residential roofing activities on a single family home.
