29 CFR  1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) PEDRO LUIS VELASCO FIERRO DBA Fierros Roofing, LLC located at 7451 Williamson Lane Canal Winchester, OH, 43110: On and preceding August 19, 2022, the employer did not initiate and maintain safety and health programs for employees removing/installing roofing shingles, thereby exposing employees to the hazards associated with performing roofing work which included but was not limited to falls up to 25 feet.

Employees removing/installing roofing materials were not provided training to control or eliminate hazards associated with performing roofing work which included but was not limited to falls up to 25 feet.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) PEDRO LUIS VELASCO FIERRO DBA Fierros Roofing, LLC located at 7451 Williamson Lane Canal Winchester, OH, 43110: On and preceding August 19, 2022,  the employer did not ensure that an extension ladder used for access to the roof of a two-story residential house had the side rails extending at least 3 feet beyond the upper landing nor was the ladder secured the top with a grasping device provided. The following condition exposed employees to falls up to 10 feet.
