29 CFR  1910.132(d)(1):The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  a) Central Machinery Company doing business as Cenmac Metal Works, located at 1339 E. Fairground Road, Marion, Ohio 43302: On or about August 15, 2022, the employer failed to ensure that a hazard assessment identifying and selecting appropriate personal protective equipment was conducted prior to the performance of metal fabrication work in Building 1, Building 2, and Weld Shop. Employees performing metal fabrication activities such as but not limited to, press brake operation and welding, without an employer assessment were exposed to eye, face and other bodily injuries.

29 CFR  1910.132(f)(1):The employer did not provide training to each employee who is required by this section to use personal protective equipment:  a) Central Machinery Company doing business as Cenmac Metal Works, located at 1339 E. Fairground Road, Marion, Ohio 43302: On or about August 19, 2022, the employer failed to provide training relating to use and care of employer selected personal protection equipment. Employees in Building 1, Building 2, and Weld Shop while performing metal fabrication activities such as but not limited to, press brake operation and welding, without the benefit of the aforementioned training were exposed to eye, face and other bodily injuries.

29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a)Central Machinery Company doing business as Cenmac Metal Works, located at 1339 E. Fairground Road, Marion, Ohio 43302: On or about August 19, 2022, employer failed to develop and implement an energy control program consisting of energy control procedures, effective training, and periodic inspections to ensure that the hazardous energy sources are isolated and rendered safe prior to employees performing servicing activities including but not limited to changing tooling on the Diamond press brakes.  Employees were exposed to caught in and crushing points in the working ara of the die and ram with potential amputation injuries in the event of the Diamond press brakes starting un-expectantly.

29 CFR  1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) Central Machinery Company doing business as Cenmac Metal Works, located at 1339 E. Fairground Road, Marion, Ohio 43302: On or about August 19, 2022, the employer failed to ensure one or more methods of machine guarding was provided on the Diamond press brakes in Building 2.  Employees performing forming work on this machine were exposed to crushing and amputation injuries created at the unguarded point of operation.
