29 CFR 1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about August 05, 2022 employees were exposed to eye injuries while decking a roof of a residential building, using pneumatic nail guns without the use of eye protection.

29 CFR 1926.405(g)(2)(iii):Splices. Flexible cords shall be used only in continuous lengths without splice or tap. Hard service flexible cords No. 12 or larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced. On or about August 05, 2022 employees were exposed to shock/electrocution hazard while working with power tools plugged into flex cord with repaired with splice that did not retain insulation.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. On or about 08/05/22 employees were installing roof decking on a 2-story residential building, approximately 10 feet above lower ground level without the use of fall protection.
