Frito-Lay, Inc. at 116745 E. 37th Ave., Denver, CO 80239: On August 18, 2022, authorized employees were not trained to ensure they had the knowledge and skills required for the safe application of the energy control program. On August 12, 2022 an employee sustained an finger amputation while conducting a maintenance on the CALJAN conveyor. This condition exposed employees to amputation hazards.
