29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On the west side of the work site, on or about August 17, 2022 and at times prior thereto, employees engaged in roofing activities on an 8:12 pitch roof without fall protection were expose to unprotected fall hazards of approximately 22-feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On the west side of the work site, on or about August 17, 2022 and at times prior thereto, employees installing roofing material used an extension ladder that did not have the side rails extended three feet over the roof surface were exposed to potential fall hazards greater than six feet.
