29 CFR 1910.178(o)(1): Only stable or safely arranged loads shall be handled. Caution shall be exercised when handling off-center loads which cannot be centered: (a) On or before August 12, 2022, the employer did not protect employees from injury in that an employee sustained a serious arm injury after being struck by a section of pipe that was being loaded onto a trailer with a forklift. Dunnage had been placed on the trailer under the pipe at one end but not under the other end. The forklift operator raised one end of the pipe so another employee could place dunnage under that section of pipe. The pipe shifted and struck the employee. This condition exposed employees to struck-by hazards.

On 8/12/22 an employee sustained a fractured arm after being struck by a section of pipe that was being loaded onto a trailer with a forklift. The employer reported in the incident on 8/15/22.
