29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  Highview in the Woodlands located at 1000 Falcon Point Place, Rockton, Illinois 61072:  On or about June 10, 2022 through August 22, 2022, the employer did not develop and implement an adequate written respiratory protection program with worksite specific procedures for respirator use that included all provisions in 29 CFR 1910.134(c)(1)(i)-1910.134(c)(i)(ix).  The employer required employees to wear N95 filtering facepiece respirators throughout their shifts, while employees were performing duties such as administering medication to residents, assisting with bathing, feeding, or dressing, on suspected or confirmed positive COVID-19 residents.   Abatement documentation is required in accordance with 29 CFR 1903.19(d).

29 CFR  1910.134(e)(1):  The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  Highview in the Woodlands located at 1000 Falcon Point Place, Rockton, Illinois 61072:  On or about June 10, 2022 through August 22, 2022, the employer did not provide a medical evaluation to determine each employee's ability to use a respirator before requiring the use.  The employer required employees to wear N95 filtering facepiece respirators throughout their shifts while employees were performing duties such as administering medication to residents, assisting with bathing, feeding, or dressing, on suspected or confirmed positive COVID-19 residents.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

The employer required employees to wear N95 filtering facepiece respirators throughout their shifts while employees were performing duties such as administering medication to residents, orally or intravenously, assisting with bathing, feeding, or dressing, on suspected or confirmed positive COVID-19 residents.   Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
