29 CFR 1928.57(a)(6): At the time of initial assignment and at least annually thereafter, the employer did not instruct every employee in the safe operation and servicing of all covered equipment with which he was or would be involved: (a) Jobsite located at 1205-1221 Newark Rd, Toughkenamon PA: Employees were exposed to amputation, crushing and caught-in-between hazards in that the employer did not implement and provide an effective training program for employees that regularly operate agricultural equipment. Condition noted on or about the 6-month period of Feb. 15 - Aug. 15, 2022.

29 CFR 1928.57(b)(2)(i): (Farm Field Equipment) The mesh or nip-points of all power driven gears, belts, chains, sheaves, pulleys, sprockets, and idlers were not guarded. Jobsite located at 1205-1221 Newark Rd, Toughkenamon PA: (a) Employees were exposed to amputation, crushing and caught-in-between hazards while operating and/or working in close proximity to a portable conveyor system in that two chain & sprocket assemblies located at the machine's control unit were not enclosed or guarded. (b) Employees were exposed to amputation, crushing and caught-in-between hazards while operating and/or working in close proximity to a portable conveyor system in that unguarded return rollers underneath a 30-foot conveyor belt were not enclosed or guarded. (c) Employees were exposed to entanglement hazards while working in close proximity to a portable conveyor system with projecting horizontal shaft ends that did not have smooth edges and/or were not guarded by nonrotating caps or safety sleeves. Condition noted on or about the 6-month period of Feb. 15 - Aug. 15, 2022. E&H Conveyors, Inc. DBA KJ Mushroom Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1928.57(b)(2)(ii), which was contained in OSHA inspection number 1523707, citation number 1, item number 1 and was affirmed as a final order on 10-25-2021, with respect to a workplace located at 1205-1221 Newark Rd, Toughkenamon, PA 19374.
