29 CFR 1926.451(c)(2)(iii): Unstable objects were used as working platforms: a) On or about July 28, 2022 at the job-site located at 5121 Hyde Park Drive, Fort Myers, Florida, an employee was exposed to a 28-ft fall hazard when standing on a job-made wooden work platform elevated by a JCB rough-terrain forklift (Model 507.2 F, SN#: JCB5CEGVJ2735137) while installing roof tiles to a residential structure. The wooden work platform was tied to the forks attachment by ropes, which showed signs of broken wood and lacked forklift pockets.
