29 CFR 1910.23(c)(9): The employer did not ensure that portable ladders that are used on slippery surfaces were secured and stabilized: Shop Floor, Spray Booth Enclosure: On, or about, August 11, 2022, a metal extension ladder slipped when in use, causing an employee to fall to the concrete floor and suffer serious injuries. The ladder was not held or secured in place. Abatement Note: In addition to providing a more secure method to access the top of the enclosure, the coefficient of friction under both dry and wet conditions should be evaluated to determine if the walking surface meets generally acceptable standards. If not, the floor surface should be cleaned/coated to prevent slips and falls in the future.
