29 CFR 1910.147(f)(3)(ii)(D):Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: a) For the asphalt plant at/near 806 NW 9th St., Okeechobee, FL 34972, on or about 08/24/2022, the employer exposed employees to amputation hazards, in that, all employees performing maintenance activities on conveyor belts were not applying their own locks and tags on electrical disconnect energy isolating devices. Rather, only one employee applied locks and tags to the electrical disconnect for the conveyor line and the other employee/s worked on the equipment without having and applying their own locks and tags.
