1910.29(b)(13)(i): The guardrail system opening used around holes that serve as points of access (such as ladderways), did not have a self-closing gate that slides or swings away from the hole, and was equipped with a top rail and midrail or equivalent intermediate member that met the requirements in paragraph (b) of this section: a) Facility, where openings at the access point of the Lapeyre alternating tread stairs not equipped with self-closing gates included: 1. Visitor Center East Mechanical Room, exposing employees to a fall of approximately 10-12 feet to the lower level. 2. Administrative Building second floor storage room, exposing employees to a fall of approximately 10-12 feet to the lower level.

Where the employer utilized the lockout tagout program for safe electrical work practices no written procedures had been developed for repairing and or installing outlets. 2. Written procedures had not been developed where employees performed preventative maintenance and repairs on gasoline and diesel operated equipment, including but not limited to, John Deere riding mower, John Deere 6430 Agricultural tractor, CAT backhoe, Yale forklift, and the dump truck.
