29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located, and identified so that they were readily accessible without subjecting the employees to injuries. On or about August 24, 2022, fire extinguishers in multiple locations in the fabrication building were blocked and not readily accessible to employees. a. On the west side of the fabrication shop behind the Ridgid pipe-cutting machine, the fire extinguisher was blocked from access by used pipe flanges and the Ridgid Pipe cutting machine. b. On the west side of the fabrication shop approximately fifty-five feet from the Ridgid pipe-cutting machine adjacent to a support beam, the fire extinguisher was blocked from access by shop materials.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one-eighth (1/8) inch. On or about August 24, 2022, on the west side, fabrication Alloyed shop employee(s) utilized a Baldor eight-inch (8) bench grinder (X1102187005) with an opening between the work rest and grinding wheel greater than a one-eighth inch to clean and smooth rough areas in the machining process.
