29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee was using eye protection when exposed to eye or face hazards from flying particles.     A) Three Guys & a Roof/Siding LLC worksite located 210 Village Rd. Pioneer, OH 43554:  On or about August 23, 2022, the employer did not ensure employees were protected from eye injuries while installing shingles on a single story residential structure using  Metabo HTP 1 3/4"Coil Nailer pneumatic nailers.  The Three Guys & a Roof/Siding LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1510182, citation number 1, item number 2 and was affirmed as a final order on March 3 , 2021 with respect to a workplace located at 4139 Bonnie Brook Rd, Ottawa Hills, OH  43606.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):        A) Three Guys & a Roof/Siding LLC worksite located 210 Village Rd. Pioneer, OH 43554:  On or about August 23, 2022, the employer did not ensure employees were protected from falls while tearing off old shingles and installing new shingles on a single story residential structure.. The employees were exposed to a fall distance of approximately 8.5 feet from the roofs edge.  The Three Guys & a Roof/Siding LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1510182, citation number 1, item number 3 and was affirmed as a final order on March 3, 2021 with respect to a workplace located at 4139 Bonnie Brook Rd, Ottawa Hills, OH  43606.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:   A) Three Guys & a Roof/Siding LLC worksite located 210 Village Rd. Pioneer, OH  43554:  On or about August 23, 2022, the employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing roofing operations from elevated surfaces greater than six feet from the level below.  The Three Guys & a Roof/Siding LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1510182, citation number 1, item number 1a and was affirmed as a final order on March 3 , 2021 with respect to a workplace located at 4139 Bonnie Brook Rd, Ottawa Hills, OH  43606.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  A) Three Guys & a Roof/Siding LLC worksite located 210 Village Rd. Pioneer, OH 43554:  On or about August 23, 2022, the employer did not ensure the side rails of an extension ladder being used on the side of a single story residential structure was extended at least three feet above the landing surface.  Employees were exposed to falls while using the ladder to access/egress from the roof.  B)Three Guys & a Roof/Siding LLC worksite located 210 Village Rd. Pioneer, OH 43554:  On or about August 23, 2022, the employer did not ensure the side rails of an extension ladder being used on the front of a single story residential structure was extended at least three feet above the landing surface.  Employees were exposed to falls while using the ladder to access/egress from the roof.  The Three Guys & a Roof/Siding LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1510182, citation number 1, item number 4 and was affirmed as a final order on March 3 , 2021 with respect to a workplace located at 4139 Bonnie Brook Rd, Ottawa Hills, OH  43606.
