29 CFR  1903.19(d)(1):The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a)  For Citation 1, Item 1 issued on 10/6/2022 with an abatement date of 11/2/2022, the employer did not submit documents demonstrating the abatement of the cited violation to OSHA.

29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about August 25, 2022, five employees were exposed to falls of approximately 12 feet and were not protected from falling by the use of a guardrail, safety net, or personal fall arrest system while conducting roofing operations on 6 in 12 pitch roof.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about August 25, 2022, three ladders were used to access upper landing surfaces at various locations along the eves of a 6 in 12 pitch roof, and the ladders did not extend at least 3 feet above the landing surfaces.
