29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. Pelham Memorial School - On August 23, 2022, each employee was not protected from a fall of over 6 feet where the safety monitor was helping install insulation on the roof deck and was not watching other employees on the roof for how close they were getting to the edge. An employee suffered serious injuries when he fell approximately 15 feet to a lower roof when he was not warned by the safety monitor about being close to the edge.

29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function. Pelham Memorial School - On August 23, 2022, the safety monitor had other responsibilities where he was helping install insulation on the roof deck and was not watching other employees on the roof for how close they were getting to the edge. An employee suffered serious injuries when he fell approximately 15 feet to a lower roof when he was not warned by the safety monitor about being close to the edge while installing insulation.
