29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard was not attached to the boom or basket when working from an aerial lift. As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). (a) On or about August 25, 2022, an employee was using a JLG aerial lift (Model No. 660SJ & Serial No. 0300293498) to rebuilding the entrance to a domed road salt storage building. Employees were not protected from an approximate 15 foot fall/thrown hazard, by using a safety harness and lanyard attached to the lift. (b) (a) On or about August 25, 2022, an employee was using a JLG aerial lift (Model No. 460SJ & Serial No. 0300248112) to rebuilding the entrance to a domed road salt storage building. Employees were not protected from an approximate 15 foot fall/thrown hazard, by using a safety harness and lanyard attached to the lift.

(a) On or about August 25, 2022, an employee was tearing off shingles on the entrance to a domed road salt storage building. The employee was wearing a full body harness but had his harness tied to the anchor point rope without a proper connector. This exposed the employee to a fall hazard of approximately 6 feet.
