29 CFR  1926.453(b)(2)(v): A body belt was not worn and a lanyard was not attached to the boom or basket when working from an aerial lift.  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).   (a) On or about August 25, 2022,  an employee was using a JLG aerial lift (Model No. 660SJ & Serial No. 0300293498) to rebuilding the entrance to a domed road salt storage building.  Employees were not protected from an approximate 15 foot fall/thrown hazard, by using a safety harness and lanyard attached to the lift.  (b) (a) On or about August 25, 2022,  an employee was using a JLG aerial lift (Model No. 460SJ & Serial No. 0300248112) to rebuilding the entrance to a domed road salt storage building.  Employees were not protected from an approximate 15 foot fall/thrown hazard, by using a safety harness and lanyard attached to the lift.

29 CFR  1926.502(d)(1): Personal fall arrest system connectors were not drop forged, pressed or formed steel, or made of equivalent materials:.   (a) On or about August 25, 2022,  an employee was tearing off shingles on the entrance to a domed road salt storage building.  The employee was wearing a full body harness but had his harness tied to the anchor point rope without a proper connector.  This exposed the employee to a fall hazard of approximately 6 feet.

29 CFR  1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.   (a) On or about August 25, 2022,  an employee was tearing off shingles on the entrance to a domed road salt storage building.  The employee was wearing a full body harness but had his harness tied to the anchor point rope at the waist of his harness and not in the center of his back.  This exposed the employee to a fall hazard of approximately 6 feet.

The employer did not ensure the step ladder was used properly, fully open with spreader locked.
