Untrained employee(s) operating the Genie GTH-844 telehandler as well as other employees working in the area were exposed to struck-by hazards.

Employees working atop the elevated scaffolding platform in this condition were exposed to fall hazards  of approximately 13 feet 4 inches to the stone level below.

Two employees operating from the elevated platform without the benefit of fall protection, were exposed to falls of approximately 13 feet 4 inches to the stone level below.
