29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level. a. On or about August 26, 2022, the employer did not ensure that an exit route in the warehouse, immediately to the right of the entrance from the offices, was kept clear of materials, including plastic totes and boxes, which exposed employees to hazards.

29 CFR 1910.133(a)(3): The employer shall ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design, or wears eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses. a. On or about August 26, 2022, the employer did not provide nor enforce the use of prescription lens eye protection with side shields for employees engaged in grinding operations, which exposed them to struck-by hazards.

Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. a.
