29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) Rubber Press #1 Production Line: On or about 8/19/2022 and at times prior, employees were exposed to caught in hazards, in that there were no guards in place on mixer #1 preventing employees access to places hands in the machine while the machine is running. Employees use the rubber press to manufacture rubber mats and the employees is responsible to ensure the rubber comes out of the chute without clumps. The employer installed guards after the incident on 8/19/2022.

29 CFR 1910.147(c)(7)(iii)(A):Retraining shall be provided for all authorized and affected employees whenever there is a change in their job assignments, a change in machines, equipment or processes that present a new hazard, or when there is a change in the energy control procedures. a) Rubber mat manufacturing facility: On or about 8/19/2022, and at times prior, amputation hazards, in that the employer implemented new equipment or changes to equipment or processes, and failed to properly retrain affected and authorized employees on control of hazardous energy procedures specific to the new equipment or process. Apache Mills was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(7)(i)(A), which was contained in OSHA inspection number 1471880, citation number 1, item number 2 and was affirmed as a final order on 6/24/2020), with respect to a workplace located at 197 Royal Dr. SE, Calhoun, GA 30701.
