29 CFR 1910.134(e)(1):The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: a)Punxsutawney Finishing Works, Inc., 857 Woodland Ave Extension, Punxsutawney, PA 15767, Abrasive blasting booth area, on or about August 26th, 2022 and times prior thereto: Employees were required to wear supplied air respirators (RPB brand NOVA loose fitting hoods) during work tasks including, but not limited to, abrasive blasting operations. Some employees were not provided with a medical evaluation for respirator use prior to the use of the supplied air respirators. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.134(i)(1)(ii): Breathing air did not meet requirements for Grade D breathing air as described in the ANSI/Compressed Gas Association Commodity Specification G-7.1-1989: a) Punxsutawney Finishing Works, Inc., 1857 Woodland Ave Extension, Punxsutawney, PA 15767, Abrasive blasting/Compressor area, on or about August 26th, 2022 and times prior thereto: Company employees were utilizing supplied breathing air from two oil-lubricated Quincy air compressors (Model: QST50ANA32S0 Serial # 99412H; and Model: SI02ANA3JG Serial#90611) through a supplied air respirator associated with a Radex airline filter system. The company did not ensure that the compressed breathing air, supplied to company employees, met at least the requirements for Grade D breathing air. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.134(i)(5)(iii): The compressor's sorbent bed and filters were not maintained and replaced or refurbished periodically as instructed by the manufacturer: a) Punxsutawney Finishing Works, Inc., 1857 Woodland Ave Extension, Punxsutawney, PA 15767, Abrasive blasting/Compressor area, on or about August 26th, 2022 and times prior thereto: Company employees were utilizing supplied breathing air from two oil-lubricated Quincy air compressors (Model: QST50ANA32S0 Serial # 99412H; and Model: SI02ANA3JG Serial#90611) through a supplied air respirator associated with a Radex airline filter system. The company's filter system was not replaced or refurbished per the manufacturers instructions. The last filter changeout for the Radex airline filter system was dated as 4/3/2020 on the Radex airline filter. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.134(i)(5)(iv): The employer did not ensure that compressors used to supply breathing air to respirators were constructed and situated so as to have a tag maintained at the compressor containing the the signature of the person authorized by the employer to perform the change: a) Punxsutawney Finishing Works, Inc., 1857 Woodland Ave Extension, Punxsutawney, PA 15767, Abrasive blasting/Compressor area, on or about August 26th, 2022 and times prior thereto: Company employees were utilizing supplied breathing air from two oil-lubricated Quincy air compressors (Model: QST50ANA32S0 Serial # 99412H; and Model: SI02ANA3JG Serial#90611) through a supplied air respirator associated with a Radex airline filter system. The company's filter system did not have contain a signature of the person that last performed the filter change out. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
