29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listed or labeling: Maintenance Shop: A 4 X 2 inch receptacle box with holes on the back side designated for fixed location was wired with a black flexible cord with plug and being utilized as an extension cord. Employees were exposed to electrical hazards.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: Ranger room: Three outlets in complete installation and energized were missing their covers. Employees were exposed electric hazard. The U.S. Army Corps of Engineers (USACE) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(b)(2)(i), which was contained in OSHA inspection number 1598276, citation number 04, item number 02 and was affirmed as a final order on 07/08/2022, with respect to a workplace located at 1160 Lake Mendocino Drive, Ukiah, CA 95482.
