29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about August 26, 2022, at a residential construction site in Katy, TX, employees were exposed to a fall hazard of more than 6 feet while working on the roof of a house without the use of of a ladder extended above 3 feet of the roof edge for access.
