29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycles: Production Area: On or about August 18, 2022, the employer did not ensure that that point of operation on the Accupress CNC Hydraulic Press Brake (MN 7254, SN 5074 ) was adequately guarded to prevent employees from accessing the area during machine cycles. This exposed employees to amputation and crushing hazards during normal operations of the equipment. Abatement documentation is required for this item in accordance with 29 CFR 1904.19(d).

29 CFR 1910.1200(f)(10): The employer did not ensure that workplace labels or other forms of warnings were legible, in English and prominently displaced on the container or readily available in the work area throughout each work shift On or about August 26, 2022, the employer did not ensure that every container containing chemicals was labeled. An unlabeled Styrofoam container filled with cutting fluid was found in the production area near the cutting oil storage rack.
