29 CFR 1910.134(d)(3)(iii)(B)(2): The employer did not implement a change schedule for respirators not equipped with an End of Service Life Indicator that would ensure cartridges were changed before the end of their service life: On or about August 8, 2022, there was a hazardous waste operations and emergency response incident in the waste-water treatment plant that services the LED Lab in the facility at 1099 Ivanhoe Road. Cleveland, OH. The employer failed to ensure that the maintenance employee/s responsible for reacting to and correcting the hazardous waste operations and emergency response incident had adequate respiratory protection. The employer provided employee/s with 3M full-face negative pressure respirator with multigas/P100 (model no. 60926-3M) vapor cartridges. During the emergency response, break-through of the respirator cartridge occurred and an employee received respiratory injuries.
