This was most recently documented on August 25, 2022 at 1401 14th Street, Beatrice, Nebraska when employees were observed working on a residential structure with a roof pitch greater than 4:12. The employees where installing shingles while working at a height of approximately 9 feet 2 inches measured from the ground to eave without fall protection.

This was most recently documented on August 25, 2022 at 1401 14th Street, Beatrice, Nebraska when employees were observed working on the roof of a residential structure. The ladder used by the employees was not extended over the upper landing surface.
