See Table O-10.  At the establishment located at 1341 South State Street, Green Bay, Wisconsin; in the Forge Shop; the employer did not provide and insure the usage of point of operation guards for each trim press, exposing employees to caught between or under hazards in the following instances:  a) On or about August 29, 2022, the Trim Press #42 (Asset G0286) mechanical press was operated without protecting an employee from the point of operation.  b) On or about August 29, 2022, the Trim Press #41 (SN 7-6998) mechanical press was operated without protecting an employee from the point of operation.  c) On or about October 12, 2022, the Trim Press P28 mechanical press was operated without protecting an employee from the point of operation.  d) On or about October 12, 2022, the Trim Press #40 mechanical press was operated without protecting an employee from the point of operation.  e) On or about October 12, 2022, the Trim Press #46 mechanical press was operated without protecting an employee from the point of operation.

This responsibility includes scheduling and recording the inspection of guards and point of operation protection devices at frequent and regular intervals. Recording of inspections shall be in the form of a certification record which includes the date the inspection was performed, the signature of the person who performed the inspection and the serial number, or other identifier, of the equipment inspected.  At the establishment located at 1341 South State Street, Green Bay, Wisconsin; in the Forge Shop, the employer did not ensure that scheduling and recording of inspections for the guarding mechanisms and point of operation guarding was performed for equipment, exposing employees to struck by, caught in, and caught under hazards.
