29 CFR 1910.215(a)(4):Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: Employees in the Maintenance Barn area were exposed to struck by hazards in that the work rest on the Dayton 7" wheel Grinder was approximately 3/4 inch away from the grinding wheel due to severe uneven wear of the wheel which created pinch points when used by the employees.

29 CFR 1910.272(k):Receiving pit feed openings in the grain handling facility, such as truck or railcar receiving pits, were not covered by grates which had a maximum opening width of 2.5 inches: At Scott Co-Op receiving area, employees are exposed to trip and fall hazards in that grate opening width is 2.75 inches.
