29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Location: 309 Beechmont Drive, New Rochelle, NY 10804 a) On or about August 29, 2022, an employee was not instructed on the unsafe conditions and hazards of a skid steer machine GEHL 6640-Turbo Model 650 and was exposed to a fall hazard when he used the skid steer to load dirt and fell out of the front cabin window of the machine.
