1926.652(b)(1)(i) Excavations shall be sloped at an angle not steeper than one and one-half horizontal to one vertical (34 degrees measured from the horizontal), unless the employer uses one of the other options listed below. Employees engaged in excavation activities at 1101 S 119th St, Omaha, Nebraska, were exposed to caught in hazards in that one wall of the excavation that was over 6 feet deep, and was not protected from a cave-in through the use of benching, sloping, or any other method listed in 29 CFR 1926.652.

29 CFR 1926.1053(b)(4): A ladder was used for purposes other than the purpose for which it was designed: Employees engaged excavation activities at 1101 S 119th St, Omaha, Nebraska, were exposed to a fall hazard while using an A frame ladder to enter and exit an excavation and the ladder was not in the opened position with its spreader lock engaged and all four feet contacting the ground as per the manufacturer's operating instructions.
