29 CFR  1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces were kept in a clean, orderly, and sanitary condition:  (a) Blaster 2 Unloading Area - Excess blast media fell off parts and settled into the porous concrete floor, causing the floor to become slippery at the unloading end of Blaster 2, exposing employees to slip and fall hazards, on or about August 31, 2022.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Near the Entrance into the Plant from the Office Area - The Armstrong-Blum Marvel number 8 vertical band saw, used to cut metal, did not have the upper band saw wheel fully encased by guarding, exposing employees to a caught in hazard from rotating parts, on or about July 31, 2022.  Abatement certification required within 10 days after abatement date. 29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:   (a) Near the Parts Oven - The employer used receptacles on a relocatable power tap (a.k.a., power strip or surge protector) to power lights and had the relocatable power tap supplied through an extension cord.  The UL listing for the relocatable power tap specifies using the device for supplying power to low amperage electronic equipment and specifies not using the device connected to an extension cord, exposing employees to an electrical hazard, on or about August 31, 2022.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
