29 CFR 1926.501(c)(1): When employees were exposed to falling objects, the employer did not have each employee wear a hard hat and did not erect toe boards, screens, canopy structures or barricades to prevent objects from falling from higher levels: a) Aviation Operations Center Building: On or about 08/31/2022, toe boards were not provided around the ladder access on the roof to prevent a possible struck-by hazard . Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.502(b)(13): When guardrail systems were used around holes which were used as access (such as ladderways), they were not provided with a gate, or were not so offset that a person could not walk directly into the hole: a) Aviation Operations Center Building: On or about 08/31/2022, the ladder access on the roof was not offset or provided with a gate to prevent employees from walking directly into the opening. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
