On 8/25/2022, an employee sustained an amputation to both legs above the knee while clogging the extruder machine.        b) Extruder Machine/Line # 8:  The service/access door 23 inches in width and 30 inches in length on the extruder machine line 8 was held closed by a wire and did prevent employee exposure to an amputation hazard while clearing a jam, on or about 8/25/2022.  c) Extruder Machine/Line # 6:  The service/access door 23 inches in width and 30 inches in length on the extruder machine line 6 was not guarded to prevent employee exposure to an amputation hazard while clearing a jam, on or about 8/25/2022.   XL Plastics was previously cited for a violation of this occupational safety and health standards for its equivalent standard 29 CFR 1910.212 (a)(3)(ii), which was contained in OSHA inspection number 146771, citation number 1, item number 1 and was affirmed as a final order on 8/19/2019, with respect to a workplace located at 220 Clifton Boulevard Clifton, NJ 07011.
