29 CFR 1910.178(q)(1):Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: At a worksite located at 1050 Northeast 27th Street, Blue Springs MO 64029, employees were exposed to struck by hazards and hazards related to the unexpected release of liquid propane. The employer exposed the employees to these hazards by allowing them to operate a powered industrial truck with a propane tank that was not securely attached.
