(a) On or about September 1, 2022, the employer did not ensure that the competent person was conducting frequent and regular inspections of the job site to ensure fall protection was being worn, that fall protection systems were installed properly, that ladders were properly maintained, and job site conditions were maintained safe.

(a) On or about September 1, 2022, employees conducting roofing activities (tear-off) on a second story 4:12 pitch roof were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately nineteen (19) feet on the North and South sides of the structure.  (b) On or about September 1, 2022, employees conducting roofing activities (tear-off) on a first story 4:12 pitch roof were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately nine (9) feet on the North side of structure.

This exposed employees accessing and egressing the roof to a fall hazard of approximately Nineteen (19) feet.  (b) On or about September 1, 2022, the portable extension ladder being used to access the single story area of the roof structure did not extend at least three feet above the point of contact  and landing surface. This exposed employees accessing and egressing the roof to a fall hazard of approximately nine (9) feet.
