29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)  (a) On or about July 17, 2024, located at 504 E Randy Road, Oak Creek, WI 53154, the employer did not ensure frequent and regular inspections of the job site were conducted by a competent person to ensure fall protection was being worn, that ladders were properly maintained and used, and job site conditions were maintained safe.  Los Gallos Roofing, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1618856, citation number 1, item number 1 and was affirmed as a final order on February 22, 2023, with respect to a workplace located at 1388 30th Street, Somers, WI 53171.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about July 17, 2024, located at 504 E Randy Road, Oak Creek, WI 53154, an employee was performing tear-off operations on a residential roof without being protected by the use of fall protection, exposing employees to a fall hazard of approximately nine (9) feet.  Los Gallos Roofing, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1618856, citation number 1, item number 2 and was affirmed as a final order on February 22, 2023, with respect to a workplace located at 1388 30th Street, Somers, WI 53171.  Los Gallos Roofing, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1732469, citation number 2, item number 1 and was affirmed as a final order on April 30, 2024, with respect to a workplace located at 6632 Tartan Trail, Sun Prairie, WI 53590.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about July 17, 2024, located at 504 E Randy Road, Oak Creek, WI 53154, the employer failed to ensure the extension ladder placed on the gable side of the garage roof used to access the upper landing extended at least three feet, exposing employees to a fall hazard of approximately 12 feet.
