29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about April 9, 2024, 5 employees engaged in residential construction activities, such as removal and installation of roofing materials, as high as 18 feet above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about April 9 , 2024, the portable extension ladder being used to access the second  story area of the roof structure did not extend at least three feet above the point of contact and landing surface . This exposed employees accessing and egressing the roof to a fall hazard of approximately 9.5 feet.   Los Gallos Roofing, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1618856, citation number 1, item number 4 and was affirmed as a final order on 2-22-2023, with respect to a workplace located at 1338 30th, Somers, WI 53171
