A) 5 Drago Way, Ossining, NY: Employees were exposed to fall hazards of approximately 25 feet while working on a pump jack scaffold that was not braced at the bottom. The employees were not wearing any type of fall protection either. The employees were exposed to falls/scaffold collapse hazards, on or about August 30th, 2022
