29 CFR 1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system. On or about September 1, 2022, at this workplace, employees conducted roof repairs and exposed employees to fall hazards at the height of greater than 6 feet above a lower level, the employees stepped on top of the parapet wall to gain roof access from an extension access ladder without a fall protection system in place.

On or about September 1, 2022, at this workplace, employees conducted roof repairs and were exposed to fall hazards at the height of greater than 6 feet above a lower level, the employees access ladder did not extend at least 3 feet above the upper landing surface being accessed.
