29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  (a) Worksite: On or about 9/2/2022, employees were exposed to fall hazards while working from a pump jack scaffold without adequate training to recognize the hazards associated with pump-jacks, to include missing bottom bracing, no fall protection, and the scaffold being erected approximately 2 feet from a power line.
